dvr recordings disappear each day then reappear the next

I'm having a weird issue with my X1 box. Random shows will disappear from my DVR and then reappear the next day. We realized that if it was the first time that day going to saved recordings, then we would see everything. If we closed out and went to the recordings 5 minutes later, random recordings would disappear until the next day. I didn't matter if we had watched them or not. We only have 3 recordings (3 one hour shows) on the dvr right now and only one recording is showing. I've had reset signals sent multiple times and it doesn't resolve the issue. 

I also started noticing that occasionally when fast forwarding at 1x or 2x speeds it will randomly skip to the end of the show and ask if I want to delete because it appears I have "finished watching". I was only 10 minutes in to a 1 hour show. Any ideas? I feel like this is most likely a hardware issue.
