closed caption problems

I also rely on closed captioning to help with my hearing problems. The text that is displayed on-screen is frequently delayed by quite a bit from the spoken words. Also, there are many transcription errors where the displayed text makes no sense. The error frequency is very high. Is CC done by a human or a computer? Either way I would like to see improvements. This happens on broadcast TV, streaming content and movies.
