QVC taking over other stations

We're seeing the QVC channel (and at other times we've seen FS2) pop up when we're watching some HD stations and even when watching recorded material.  We have already had the X1 box swapped out but the problem persists.

We're looking for a root cause since we don't think the problem is in the box, unless there's a software problem common to all the boxes we've had.
