Sound Sync and On Demand Issues

Hello - has anyone had issues with the sound not syncing with speakers mouth movement?  My sound continues to be delayed and it looks like whatever I'm watching is dubbed - it's very annoying.  I have techs out, changed boxes multiple times, etc.  

 

Also, not sure if it's related, but my On Demand drops sound and sometimes fast forwards on it's own.  we have not been able to watch much On Demand at all.

How is the sync when watching NBC HD local channel 'live'? what hardware are you using as a set top box? (X1, non-X1, a streaming device, etc.)
