Problems with Reward Redemptions

This is the second time. I  claimed the rewards( $10 Domino's egift card & $10 Amazon book shelf). This is horrible.  I have been a long time customer.  I am a Diamond member.  I would like my rewards. Please send me my e gift card and code for the Amazon bookshelf.  

I am very disappointed and upset over the issues on the rewards. I claim them and don't get it.
