Completing live agent evaluation that did not display at the end of our conversation

I am looking for a way to provide both an excellent evaluation of a recent live agent chat for my account at ending with  ...xxxx xxx 5076 9980 at address 1016 W Houghton St Santa Fe NM 87505  The usual pop up survey did not start and I want to make sure that is excellent work gets credited.   Glen Duval
