I am having this same problem. Mainly when I first turn the TV on for the day some apps do have volume and some do not. I have an LG and it does a sound diagnosis which tests the volume, and it performs perfectly for the test but when I try some apps I get picture but no sound. If I go to the xfinity app this will work fine but others no.
I will get sound after about 2-3 minutes of trying then it is good for the rest of the afternoon and evening. This has been happening for a few weeks now.
Thank you for your detailed post, @user_b21486. That is odd to have the sound kick in after a few minutes. Are you using the TV speakers, or, do you have other Bluetooth speakers connected?

@user_b21486 Thank you for confirming that. I just want to clarify, you mentioned you are using an LG TV and the Xfinity app works fine but other apps aren't having sound. Can you confirm if you are using a cable box or just your smart TV with the Xfinity Stream app please? I want to make sure we're troubleshooting correctly.
