Audio on On demand and DVR is all over the place.

Nothing I have done fixes it. One second I can barely hear it, the next it's blasting my ear drums from across the room. This happens with everything. I could watch something live and it will be fine, but when I watch the same thing on demand, the audio issues arise.

What is going on with this?

is the set top box connected directly to the TV and you are playing the audio on your TV speakers?  Do you use a sound bar or receiver too?

Using the TV's speakers, all connected via HDMiI.

then you are hearing studio-set (production) audio levels as they were created. like the video stream it is delivered digitally unchanged from the creators/publishers. Could your TV have an audio setting turned on causing it? some kind of audio automatic level control?

Not sure how fancy a tv you have, might be same as a problem I had. Mine goes to my 5.1 surround sound receiver over HDMI. I had it set to DTS or some fancy audio processing mode. Sometimes real load, others quiet, guess is signal randomly had output on a valid channel, and sometimes it tried to put sound on a missing speaker (i.e. 7.1 vs 5.1 surround?)
Changing my receiver to Dolby surround it stopped going in and out.
If your TV has audio modes, try a different one... maybe...
