"This information doesn't match our records" issue

Hi,
I'm trying to set up a new account for internet service, I was most of the way through the process when I got an authentication error. Now every time I try to login on the customer site it reloads the auth page as if I haven't logged in. When I try to use the "Create Account" option, even with an alternate phone number to try to start the process fresh (even after clearing cookies) I enter the 2fa code sent to my number and I get the following error dialog:

I'm not sure what's going on but it's keeping me from getting my new service ordered. Please help!

I would like to see if there was an account created or pending on our side to help smooth out the process. Could you please send us a direct message by using the "Direct Message" button that should be located to the top right of the forums page.
