A PUK (Personal Unlock Key) code is required when a SIM card is locked after entering the wrong SIM PIN ten times. This appears when you locked the SIM to the device using the integrated settings. We recommend trying code "1111" which is set as default or attempt any PIN you may have created when you processed the SIM lock on the phone.
