Problems Not Resolved by Chat

I have 2 problems.  

1) Phones (Voice hardwired line [Edited: "Personal Information"]) only ring once; callers say get dropped; phone dead by time I can answer; calls not also forwarded to another number/line.  [Can't answer my phone and also not being forwarded].  Calling out, home Internet, etc. all work fine.

Last night, I wasted over an hour hanging on line while chat agent said was testing and fixing my phone, but she just disappeared, never told me if phone problem was resolved.  Has the problem been fixed or will it be fixed?

2) I need Call History from Dec./Jan. to present.  Online only has back to late May.  How do I get, request, etc. copy of older call history?   Chat agent never addressed this before disappearing.
