Have not received refund

Hello. I am supposed to receive a refund for an account which I have closed. I have been trying to get this resolved for 3 months. I have been told multiple times that I would be sent a check, and I have not received one. I have been told that the refund was deposited into my bank account, and my bank assures me that is not the case. I have been told that I received a refund card, and I have not. This is all contradictory and cannot possibly be true. 

I would like this to be officially looked into. When I talk to agents all they say is that they will file an escalation ticket and then they all end up being closed. Nothing moves forward.

Please help me to get this resolved. I would like my refund.
