Did not have opportunity to add promotional code to order

I signed up for new service about an hour ago. When I was finished, I realized I never had the opportunity (that I know of) to add this promotional code that I was offered.

Below is a screen shot of it.

I signed up for 1 gig plan.

Would appreciate it if I could get this code applied
