adjustment-request for sporting broadcasts from April to June 2020

Where and how do I find the adjustment - request form for the sporting eventd and broadcasts from April to June 2020 as stated in the letter that I received on October 20, 2020.  I would like to make my adjustment request so that I can receive my adjustment to my bill.

Courtesy adjustments were recently issued to impacted active customers and would be visible on recent statements.

Looks like you may have moved or changed account numbers over the summer.

Im wondering...i received a letter from Xfinity saying my adjustment is $10.98 but on my invoice i was adjusted $3.38. Also I cannot fine anywhere on xfinity website to request the adjustment. Please advise
