Dispute Charges

I would like to dispute the charges on my bill.  To make a long story short, I have been charged a service call and for 2 extra TV boxes that I don’t have.  I would like to be credited.  I have discussed my issue with employees at the local store and regional managers (also online chat) and they all think I deserve to get a credit.  But nothing is done or at least communicated to me.

 

How do I formally dispute the charges on my bill so I can tell the complete story to someone who can provide complete customer care service to a loyal customer of over 30 years?
