Early in March I spoke with customer service about my billing. Long story short, he said he'd take care of things and sent me a consent form to sign to electronically sign, which I did. I recall reviewing it and all looked good.
Fast forward to today, it looks like my monthly billing is still incorrect. I am trying to find a copy of the consent form I electronically signed but the old link has expired. Could someone point me in the right direction? Specifically, I'm looking for the one that goes over my service plan and billing, not the ones that go over autopay and such - I found those.
