Intermittent issues as you have described are often the most difficult to diagnose because they are due to something as silly as a loose connection *somewhere*. That *somewhere* is the real wildcard. And it may not be a loose connection. It could be an oxidized connector that's not obvious to visual inspection. A faulty PCB in an amplifier *somewhere* along the line. Any number of things.
