I have been having the same issue. I just received a new Gateway today (400mbs plan), but the issue still remains. Occurs on both PC and Iphone Stream App. Super frustrationg.

Same problem here with the Stream web site, the mobile Stream app (Android), and the Roku app. Can view DVR recordings but all of the live channels claim "subscription required" (error code 12012 or TVAPP-00203). This seems to be affecting many people over the last week or so - so probably a back-end/server issue that Comcast needs to resolve.
