X1 equipment exchange

X1 equipment exchange

I have received notice of need for equipment exchange to upgrade to X1. The site to submit for the new kit takes you to the end. When selecting submit, there is an error which says to reload.

This is an endless loop over multiple days of trying and multiple tries per day. Please advise.
