VISA Card Help

I signed up for a student plan a while back and had issues activating after I moved addresses, I spoke to customer service and they put me on a plan that included a 250 VISA card and helped me activate it that week. I've been living here for months and I haven't gotten anything and it says I do not qualify because I never activated my services? But I have (months ago) and nothing has come in the mail.
