Comcast failed to disconnect services when asked

When someone moves and asks for their serves to disconnected by comcast and that doesn't happen what do you do? The services were supposed to be shut off the end of July and now the bill reflects the time in between and being told that there is a $300 shut off. There is proof of moving at the date stated to shut off.
