Request to change my name in ( Account holder's name)

 i requested to change my name, then sent me an email to fill out the Account Change Request Form. Then, attach and submit the form along with any supporting documents I did that , and then i got another email they need more information in order to complete my request this information is :

  • Consent to transfer account form for current account holder (page 2)
  • Consent to transfer account form for new account holder (page 3)
  • Manual Signature required on affidavit, please sign and resubmit
  • Please check the box on page 3 of the New Account Holder Xfinity Consent to Transfer Account
  • Valid Government Issued Picture Identification for Current Account Holder.

when they sent me this email , they did not send any link or forms to filling out .

and also i become confused , because these additional information the need  is for to transfer the account . How do i get this form to send it back to them ?

i went to Xfinity service center in person they said my name was changed but when i checked my account on my end the Account holders name still in my old name
