Account Change Request Form
I am trying to change the ownership of my account. I opened a ticket over a month ago. I am still being requested by the people handling my ticket to send page 2 of the Account Change Request From and an Affidavit. I have checked my email and requested the form be re-sent to me twice over the phone and it has not happened.
