Account Change Request form

I am in the process of an account change. I need to fill out the account change request form and upload it to the ticket but I was never sent the form. Where do I get the account change request form?!

Yes, I have a ticket but I never received the actual form. I received an email saying I have the ticket. I can see there is a ticket open. I've been receiving emails that request I submit the form but I never received the form.

 Thanks for clarifying, have you checked your other e-mail boxes to see if it's went to a junk file or a spam folder?

Yes, I check the junk and spam folder. I followed up with a rep on Saturday 12/14 to request the document again. I was told it would be sent to again and to wait an hour. I still have yet to receive it.

I have also requested a Account Change Request form and never received it. I checked all the folders in my email and there is nothing there. I even requested to send the form by a text message and still nothing
