Someone enabled a SIM PIN on the phone. If you enter the SIM PIN incorrectly three times you need to get a PIN Unlock Key (PUK) from the credit card sized plastic card that came with your SIM, or maybe from your cellular carrier. You can usually get this by logging in to the carrier's website, or just call the carrier and ask. The alternative is to get a new SIM card and NOT enable the SIM PIN.
