Credit for phone payoff

We switched from Verizon when Xfnity promised to pay up to $500 for each phone pay out (in the form of gift cards).  We have the invoice from Verizon that shows that we paid off the balance on the phones.  We are having absolutely no luck finding out where to submit the bills to at Xfinity.
