Residential sales tax certificate

I have been trying to access the residential sales tax certificate so I can file it with XFinity.  The closest I came was when another customer asked for this and was directed to DM.  I tried that and was admonished for using it, then ignored.  Is there ay way to access this form and, if so, where?  Also, this seems to be a new thing since none of my prior bills show this tax.  Is it legal in Colorado to add a monthly sales tax fee to these services?

Okay, so I am barred from using DM.  But I still have not gotten any assistance from XFinity with my question.  Where do I access the sales tax exemption form for residents and how do I file it with XFinity?
