Images in my questions do not show up - why?

Here is a sample image that I'm trying to include in my questions. There is nothing sensitive in it but my questions have a triangle image instead of my original photo show up. Why is this?

I added this by clicking on the camera icon in the menu, browsing to the image, selecting it, waiting for the upload to complete, then click the done button. The image shows up at that point where I'm expecting it but as soon as I post the image changes to the triangle image - including if I later go back and look at my saved images.
