Utility box caused cracked driveway. Cannot locate where to file complaint

My driveway has a Comcast utility box in the middle and the concrete in the entire driveway is cracked and is now causing a tripping hazard.  This is not a new install but the driveway has saw marks where the utility was added after the concrete was poured.  I am requesting Comcast to correct the sinking utility access in my driveway and replace my damaged driveway.
