How to cancel my service

My Billing cycle ended on April 17th and on that day i contacted Comcast to cancel my service. I was told that they would mail me prepaid boxes to return my equipment, and I thought all was going as i was instructed. I had failed to receive boxes to return my equipment, and my phone number was to be ported from comcast to att and that had not happend either. I contacted comcast again and this time I was told that they were sorry and will correct the problem, yet it requires a live phone call to verify that i am canceling. I made the phone call to verify i am canceling service and also porting my phone number over to att. This took 5 different people to get my service canceled and my number ported and a confirmation number. That was on April 20th, and it is May 6th, and my service has not been canceled or phone number ported or boxes received to send back my equipment. 

What does a person need to do to cancel comcast?
