How can I get this form from Xfinity "Account Ownership Change Request Form"?

Hi there. How can I get this form either emailed to me or send to me? Please let me know as I am moving out of my residence and need to transfer all of my Xfinity service to the person staying in the home.

You can go to a store with the person that will be taking ownership of the account. This is probably one of the fastest ways to be able to do this. Make sure the account is up-to-date on payments before making the change and make sure you have ID's showing the address of the account.
