Device payment statement

I been trying to get a statement which reflects my device payment from the support team for a while and till date I did not receive it. Went to the store multiple times and called the customer service team multiple times but no use. Need help with this immediately as I need to submit this statement by today eod to my new carrier to Get it reimbursed. My phone no is [Edited: "Personal Information"]
