Cut cable

Hello, 

we are a contractor that contacted 811 for a locate. There were several locates, but none for the location we cut the cable. All service was lost, so we spliced a standard size cable to restore service. It needs to be repaired properly with a larger coaxial cable. 
How do I report this?
