Can't submit equipment upgrade order

I am attempting to submit an order for my father-in-law (he doesn't have a computer) to upgrade his Xfnity equipment, and have tried multiple times on various devices and pcs, but continue to get the "Looks like something went wrong' message. What's the magic code to actually get this done?
