Calling to activate modem

When I call to self-activate a cable modem, what information are they going to ask for to verify the account?  I know I will need the mac address but what else do I need to give them?  I have to get the account owner (my dad) to provide all of this stuff to me ahead of time so there are no issues.

Note: I will only have a cell phone with internet during this process
