Adding TV after seasonal internet

I currently have my internet on seasonal hold. That ends next week and I want to add TV. Everytime I attempt to add TV it gives me 4 addresses to select from. Bottomline it that I cannot add TV online. Cannot find a contact number to get assistance, so thought I would try here for suggestions.
