Unreturned equipment charge

I keep getting an unreturned equipment fee charge on my bill every month. Over two years ago, a tech came and changed out my modem with the new one and took the old one back. Ever since then I’ve been getting an unreturned equipment fee Charge. No matter how many times I call in or talk to a representative. And they remove the charge. It’s back on my bill next month. How do I remove this permanently? Is this a scam? What’s going on?
