One-time charge beginnig with August and non-notified additional charge to my Xfinity package???

I did not realize my bill payment going up with an additional charge of the so-called name "one-time charge" since I am with a monthly automatic payment. My original package is 69.99 for the Pro Intenet and a fee for modem rental of $14 monthly. On July 29, a $25 Echeck/eft Return Fee was added to my bill and then my Xfinity package was charged $99.99 monthly with no notification to me or any consent from me to take this quote. What is going on here? I have to turn off the automatic payment setting in case of any further unnotified charges. I called customer service today but was not transferred to an agent to figure out this question. Seriously, I need a refund back!!

@EG​ How can I directly talk to the Billing-related employees? every time I call will end up replying with automatic answers from the robotic agent. The option provided by the call is only limited to the bill payment and due amount.
