Inconsistent Account Name and Name shown on billing statement

Hi, Please contact me at [Edited: Removed personal information.] My account name is inconsistent with my Billing statements. Please for both May and June statements and onwards. Please also put me in touch with an agent. I have not been able to fix this issue since January.
