I need a refund for equipment rental I never had.

I've spent HOURS on chat and have gotten nowhere. I discovered that I have been charged $15 a month for rental equipment with no serial number or model number assigned to it. I didn't catch this because it is not itemized on my monthly bill. This has been happening since 2019! I need the corrected and refunded in full.
