courtesy adjustment reimbursement for regional sports fees

I just received a letter from Xfinity stating that I should get a courtesy adjustment reimbursement for fees regional sports charged us during the hiatus from April to June.

I was told to to go to:  xfinity.com/adjustment-request to fill out a form to complete this request.  Where can I get this form?

Thank you for posting to the Xfinity Forums regarding the courtesy adjustment for the Regional Sports Fee. At this time, there is no action you need to take to get the credit. If owed to you, you will see it on your bill. I'd be happy to review your account to see if it's been issued or which bill you can expect it on. Please send me a private message with your full name as it appears on your billing statement. To send a message, click on my name "ComcastChe," then click "send a message.

NO!  You should NOT post private/personal info on this Public Forum.  You should edit it out and click on the message icon to send a PM to @Xfinity Support with your info for security purposes.  As @ComcastChe stated above:
