Billing increase

My bill is almost a car payment and I have called 3 times and received no help in lowering it - I can switch to another provider with more channels for 1/2 the cost. Has anyone had any luck getting a lower bill? Can I switch to the Triple Play promotion? I have been a customer 15+ years and not getting any help. Also - how do you reach a manager/supervisor who may be able to do more?
