Reprovisioning Modem?

How do I go about getting my modem reprovisoned? I'm stuck in an activate service loop, and other users have reported that as being the solution if resetting/hard resetting my router and modem isn's successful.

I would try restting the router with the reset button first.  Then just see if you  swap for another at the service center

The modem is brand new and has already been reset several times.
