Modem lost connection. Have to power off, unscrew and screw back the cable connection, power on..

Modem lost connection every few days. The current way I am doing is to power off, unscrew and screw back the cable connection, then power on. Doing on power off/on only does not solve the problem. Have to unscrew/screw back the cable. I am using netgear CM700 for about two years and this issue started two months ago. What is likely the problem?

Please post the *Downstream Power Level*, the *Upstream Power Level*, and the *SNR* (Signal to Noise Ratio) numbers as well.
