Arris Surfboard SB6190 keeps disconnecting

Hello, my SB6190 modem keeps disconnecting multiple times throughout the day, resulting in loss of internet service for a few minutes each time. I've verified that no wider outages are taking place at these times, and I've tried power cycling the modem and resetting it to factory defaults,  with no improvement. I've verified there is only one cable splitter in line before the modem, and I've cleaned the connections and verified their tightness. Here is the log and details from the modem. 

 

Appreciate your help in resolving this issue.

Most downstream channels are close to the low end of the acceptable range and you only have one upstream channel and you should have 4, possibly 5. Take your modem to point of entry for your cable and take levels for comparison purposes

Interesting. I plugged in the modem at the point of entry, where the coax splitter normally connects and here are the modem status results.

The modem disconnected again during the night, as well as this morning. Current log and status show 2 of the upstream channels dropping out and multiple T3 timeouts today. Any suggestions?
