Paying for mobile and phone disabled

I made several calls and have been pay for insurance. No service since May. Never received sim card in mail, asked to changed number was ignored, several hours on phone being transferred...apple person was useless. I am hearing impaired and paying for Iphone 8, egibile for upgrade, still can't get into account and all mobile cares about is the last 4 of my card on file. Promised 1 month would be only $60 but it's going on 3 months...phone is disabled, items running in background that I can't see...only tower in area is Verizon which garbage. I want my phone replaced asap with new sim card . Call via mobile counts as Apple care(non). I want my new phone mailed to my address original phone was mailed to. I am in rural area..find a mobile store...apple store...
