On Demand not working through cablecard, Error CL-13

I signed up for a different package back in April and since then I've been unable to view On Demand content on my Tivo (uses cablecard) and get error CL-13. It worked fine before I switched packages.  From what I've read this error is caused by the account not having the "cable card rate code" applied, so I think this is all that needs to be done. Some assistance would be very much appreciated. Hopefully a simple fix.
