Advanced call forwarding no longer works

Yesterday I had a tech here because of internet problems. He ended up swapping out my old modem/router for a new one.

When provisioning the new one, it also moved my phone service over to the new modem. ( From a seperate phone modem)

Because my alarm system uses the old phone modem, he called in a request to have the phone moved back to the old modem.

Now I have lost my call forwarding and an unable to set it back up. Tried using the Connect app and online and neither option lets me set in up.

Anyone out there that can help? I really need this.
