installing a new modem

I picked up a new modem and installed it, but something is wrong, I can't get into the internet and my phone is not working.  What am I doing wrong:?

Are the "US", the "DS", and the "Online" indicator lights lit steadily ? If so, did you ever have it activated for service ?
