Server blocking access to email

Every time I try to use email on Microsoft Outlook I get a window asking me to enter my name and password with the server address listed.  I enter the correct password (I've tried two) and get no access to sending or receiving emails.  Why

Are you using the stand alone program or Outlook 365 or outlook.com?  If the stand alone program did you set it up with all of the information needed for you to send and receive email?
